Health Information Management (Medical Records)


St. Joseph's Hospital and Medical Center's Health Information Management Department maintains all patient medical records. The medical record is a permanent document of the history and progress of every patient's medical care. The record is a compilation of observations and findings recorded by the patient's physician and members of the hospital healthcare staff, the entries and reports originate at various points in the Hospital. Physicians' dictated medical records are transcribed and added to the medical record.

Records are used for continuity of patient care, verify insurance claims for medical care rendered, as a legal/business document outlining the course of a patient's medical care, provides statistical and factual information for hospital administration and medical staff committees, for hospital accreditation, licensing and other regulatory bodies and medical research.

Patient’s have the right to receive a copy of their medical records or inspect them. Patients will need to request their records in writing. Please see the linked forms, English or Spanish, to the right. You may print a copy of the form, fill out and return to Medical Records. Medical Records will verify your identity by photo I.D., signature, or pertinent questions. We do this to protect the privacy and confidentiality of your records. Medical Records has 30 days to respond to your request and make your records available to you. If you have any questions please contact Medical Records at the number listed below.
