Geriatric Assessment Clinic

The Geriatric Assessment Clinic is dedicated to providing a comprehensive integrated evaluation of elderly patients, ages 65 and older. Assessments are performed by a multidisciplinary team of geriatric social workers, a geriatric nurse practitioner and board verified geriatric medicine physicians.

Consultations are performed for evaluation of medical and psychosocial illnesses unique to the elderly such as:

• Loss of independence
• Loss of function
• Dementia
• Falling
• Several other complex problems.
