St. Joseph's Hospital and Medical Center is committed to providing access to patient care regardless of your ability to pay.  If you do not have health insurance, our financial counseling staff will help you find government or private programs that may assist you. If you have been seen and are uninsured or underinsured, or do not qualify for any state or Federal Programs, you may be eligible for a discount on your hospital bill.  This process requires an application to be completed and a financial screening to determine the level of discount.  Our customer service agents are available to assist you with any questions you may have related to the application and Payment Assistance Program.  To apply, click on the link below to download an application.  A Customer Service agent is available to assist you with any questions you have by calling (602) 307-2965 or toll free (877) 877-8345.
